Bengaluru, Karnataka based Ubiqare Health Pvt Ltd is a healthcare start-up company that pursues a vision of making healthcare ubiquitous, easily accessible, collaborative, and smart. Ubiqare has been founded by 5 accomplished senior professionals – 3 technologists from IITB and 2 healthcare leaders with Sundararajan (Sundar) Srinivasan being the Chairman and MD and a co-founder of Ubiqare Health Pvt. Ltd. They envision themselves as enabling specialists and hospitals to extend their follow-up care to their patients after they get home and achieve the desired treatment outcomes they want to see in their patients. To the patients, we will be the all-pervasive healthcare enabler for follow-up care to receive continuity of care from his trusted specialists/ doctors from the comfort and convenience of his home. They have launched our Minimum Viable Product (Platform + Service) in Jul 2018. They claim to have served 88 patients so far (all paying), with high severity care needs, suffering from cancer, stroke, MS and post-operative rehab. We have delivered around 2800 patient-days of care and have clinically managed 2 episodes/patient/month at home. They have thus validated the demand, proven the efficacy of the care model and have underpinned a valid basis for their business plans. They combine telemedicine, clinical network of proximity centers, and home healthcare to offer at-home care service & supportive care in Bangalore to post-acute and chronic patients at home.
